Before our first scheduled visit it is important that you read and sign several documents. If you do not have a printer, please call me before your appointment and I can either mail them out to you or have a copy waiting for you to fill out before our first visit. Otherwise, please follow the instructions below and bring the signed documents with you at the time of our first appointment.

    Please print, fill out and sign and bring this document with you to our first appointment.

  2. CLIENT INFORMATION BROCHURE
    Please print, read, sign and bring this document with you to our first appointment. This is an important document that describes my practice and policies as well as your rights as a client.

  3. CONFIDENTIAL CLIENT INFORMATION
    Please print and fill out this document and bring it with you to our first appointment.
